Note: Rates quoted above are based on a 12-month agreement. Services billed monthly are paid via an automatic payment method. Patients who do not agree to automatic payments accept a 20% increase of each total invoice amount for additional billing processing. A three-month, non-refundable commitment fee equal to and counting toward the first three months of membership due upfront and in full at the time of registration.
